Custom item texture. Make it look like something else. And have that something else have like a 1-10 status bar.

You can click on the helm in order to pick up your real helm and then see its durability. Once you move again it'll go back to your cosmetic helm.

Couldn't this work if you were to use custom textures/models and custom packets? You already use a custom resource pack and send custom packets for textures, or rather to prevent custom textures.
